Beginning the Journey with Compassionate Care

Step : 1 Free In-Home Assessment (45-60 minutes)

A member of our clinical team, often a Registered Nurse, will meet with you and your family at home. This is a no-pressure conversation to understand your loved one’s medical history, memory care needs, personality, and routines. We'll assess home safety and answer all your questions to determine the best path forward.

Step 2: Personalized Memory Care Plan Development (1-2 business days)

Using the insights from our assessment, we create a detailed, customized Memory Care Plan. This plan outlines specific goals, daily activities for cognitive engagement, safety protocols, and the schedule of care. We review this plan with you thoroughly and make any adjustments until you are completely satisfied.

Step 3: Caregiver Matching & Introduction (2-3 days)

This is a critical step. We carefully select a caregiver whose skills, experience with dementia, and personality are a strong match for your loved one. We arrange a pre-service introduction to ensure everyone feels comfortable and confident before care begins.

Step 4: Care Delivery & Ongoing Communication (Daily/Weekly)

Your dedicated caregiver begins providing compassionate support according to the approved plan. We maintain open lines of communication with regular updates to your family. Our nursing team conducts regular supervisory visits to ensure the highest quality of care and to adjust the plan as needs evolve.

What Families Are Saying About Heart of Gold

5/5

Heart of Gold caregivers do such a great job caring for my 98 year old mother. They are so kind, compassionate and gentle with her. I know that my mother is in great hands with them and I never have to worry about anything. My mother said she is very blessed with the wonderful care all the caregivers provide from Heart of Gold! Thank you Heart of Gold!

5/5

I’ve had my mom with them since since 2020 and have always felt she’s been in great hands. The staff goes above and beyond and anytime I have questions, the office is always super helpful and attentive, would highly recommend them to anyone in need of a home health service.

5/5

The Heart of Gold caregivers that took care of my mother were truly exceptional. Their loving care and attention enabled my mother to remain living in her home. Whenever I visited, my mother always looked very well cared for and spoke very highly of her caregivers who also became her friends. When I left, I knew she would be safe and that was a big relief for me. When my mother, at age 98, passed away her main care providers attended her funeral. We all hugged and said good-bye. It showed me how blessed my mother was to have been surrounded by such wonderful people.
