How to Get Started with In-Home Health Care
Step : 1 Free In-Home Assessment (45-60 minutes)
A registered nurse visits your home to evaluate care needs, discuss medical history, assess home safety, and learn about your loved one's daily routine and preferences. We answer all questions and provide honest recommendations—no pressure, no obligation.
Step 2: Personalized Care Plan Development (1-2 business days)
Our clinical team creates a detailed care plan outlining services, visit frequency, caregiver qualifications, and specific care tasks. We review the plan with your family, incorporate feedback, and finalize goals. You approve everything before care begins.
Step 3: Caregiver Matching & Introduction (2-3 days)
We carefully match your loved one with a caregiver whose skills, personality, and availability align with your needs. You'll meet the caregiver before the first visit to ensure a comfortable fit. If it's not the right match, we'll find someone else—your satisfaction is guaranteed.
Step 4: Care Delivery & Ongoing Communication (Daily/Weekly)
Care begins on your preferred schedule. Caregivers document every visit in our system, and families receive regular updates via phone, email, or our secure portal. Our care coordinators check in weekly, and our RN supervisor conducts monthly home visits to ensure quality and adjust the plan as needed.
